New Clients:

• If this is your first visit, you’ll receive electronic intake and consent forms after booking.

• Please complete all forms provided via e-mail before your appointment to ensure we have all information to discuss your goals and create a customized treatment plan.

Cancellation Policy:

• We kindly ask for 24 hours notice if you need to cancel or reschedule your appointment.
• Appointments cancelled with less than 24 hours notice may be subject to a cancellation fee.

Late Arrival Policy * Please arrive on time for your appointment. * Clients arriving more than 15 minutes late may need to have their appointment rescheduled or cancelled to respect the schedules of other clients.

Please note, a non-refundable deposit is required to secure all appointments!
